Living Well Advocacy Service

The Living Well Advocacy Service supports people living in Wiltshire with any long term health condition to complete a Living Well Plan.


What is a Living Well Plan?

A Living Well Plan is a record of your hopes and preferences for the future. It allows you the opportunity to reflect on your past, identify things that are important to you today, whilst expressing your hopes and wishes for the future.

How will it help?

Completing a Living Well Plan can:

  • Encourage you to plan for the future
  • Give you confidence in what may happen in the future
  • Help others to understand you better
  • Help others who support you in making decisions, when you are unable to do so yourself

The Living Well Advocacy Service is open and here to support people living in Wiltshire.

We are open to receive new referrals for anyone wanting to access our service.  We are now able to offer visits so please do get in touch.  All our visits adhere to the government COVID-19 safe working guidelines.
