The Living Well Advocacy Service supports people living in Wiltshire with any long term health condition to complete a Living Well Plan.
A Living Well Plan is a record of your hopes and preferences for the future. It allows you the opportunity to reflect on your past, identify things that are important to you today, whilst expressing your hopes and wishes for the future.
Completing a Living Well Plan can:

‘It has helped us to get some clarity on difficult issues in a delicate, sensitive way’
‘Being able to tell your life story is very powerful; it validates everything you have done in your life and you can think to yourself – I’m alright!!’
‘I am less anxious since doing my Living Well Plan’
