The Living Well Service supports people living in Wiltshire with any long term health condition to complete a Living Well Plan.
A Living Well Plan is a record of your hopes and preferences for the future. It allows you the opportunity to reflect on your past, identify things that are important to you today, whilst expressing your hopes and wishes for the future.

We are open to receive new referrals for anyone wanting to access our service. We are now able to offer visits so please do get in touch. All our visits adhere to the government COVID-19 safe working guidelines.

Our Living Well Advocacy Service supports people with long term or progressive health conditions to create a Living Well Plan to ensure their views and wishes are known and can be acted upon – enabling them to retain control when they are no longer in a position to express their wishes.
