The Dementia Advocacy Service supports people with a concern about their memory or a diagnosis of dementia, to complete a Living Well Plan.
Our project covers Somerset, Wiltshire and Bath & North East Somerset.
A Living Well Plan is a record of your hopes and preferences for the future. It allows you the opportunity to reflect on your past, identify things that are important to you today, whilst expressing your hopes and wishes for the future.
Completing a Living Well Plan can:
• Encourage you to plan for the future
• Give you confidence in what may happen in the future
• Help others to understand you better
• Help others who support you in making decisions, when you are unable to do so yourself

Our Dementia Advocacy Service supports people with memory loss and in the early stages of dementia to create a 'Living Well Plan', as well as people in the later stages of their illness to ensure their views and wishes are acted upon – enabling them to retain control when they are no longer in a position to express their wishes.
